Doctor; I want to look younger but fillers make me heavy on the lower part of my face. Do you have any suggestions?
It is infrequent and almost a daily repeat scenario when patients present to our clinic to request a filler for their lower face. Almost all of them point to the area around the mouth and say: “Doctor, I don’t like this area I have had fillers for the Nasolabial Folds before why don’t we do the same?” My evaluation would almost always reveals the same observation which is that most of these clients are actually unhappy about the sagging fat pads and skin around the mouth rather having a real volume loss.
When we were younger, our face is closer to a “V” or heart shape, but as we get older, and with the loosening of the soft tissue ligaments, the shrinking of the fat pads, and the stretchiness of the skin after the age-related depletion of collagen and extracellular matrix, our face starts appearing as an “A” or square shape with a wide and heavy lower mid face.
Most of the practitioners in this field are used to remedy the problem with Fillers which is truly a great temporary cover up for the problem, but what happens is that in the extreme cases of skin sagging, a more than usual quantity of fillers is needed in these areas which will make the client appear unnatural and very heavy in the lower face, simply put they end having an unsatisfactory results.
My answer to these problems is always the same: “We can add fillers if you desire, but your real problem is not lack of volume but rather the falling cheek, and the prominent Jowls and adding fillers will masquerade the problem but is not the correct solution. When things shift down, the proper way to address this issue is to put them back where they belong”. A Boca Raton facelift surgeon may be required.
